There is a Snopes article on the house at 5028 New Bridge Road in Fayetteville, Arkansas. Short story: built about 2003, eventually moved sidewalk closer to street and raised grade of street.
not sure how thick the mix was when they poured the original driveway - maybe gravity took a day off?
