For diagnosis purposes check both of the shoulders for square relative to the tenon and to the stock. Maybe only 1 cutter is off. Also, maybe check the resulting tenon without pulling the stock back...
It's a shame about the first top. The way you arranged the grain is gorgeous.
Since you are in Japan, my first thought is to try a tatami mat. 50-70mm of dense organic matter should absorb vibration and be sufficiently firm to work on.

I would use an RPC for the Maka. The Maka motor cycles on and off for each mortise and might be tricky doing that control from a VFD.
