I've got to make slots in my new 3 foot wide outfeed table top for the mitre gauge & cross cut sled.
Should the table be centred to the blade? Or the majority of the table to the left or right of the blade?

I think that's really up to you. It depends on a few things - the size of the table, the width of your saw, the kind of cuts you make.

Try stickin' it in one position and practice run some common cuts you'll be doing. If you cut a lot of sheet goods, muscle up a hunk of plywood onto that thing and make a few common width cuts that youll be making. Generally, there's never a single rule for things like this - it's all dependant on how you work and what you make.

That is totally up to what sort of work you 'mostly' do. If you remove a lot of small pieces from large panels you will want support to the left. If you cut a lot of larger panels to size, you want support on the right. If you rip a lot of stock, you want centered support.
