Hello All,
Would any of you that take orders at craft shows for picture ornaments and or picture engraving on marble be willing to share your order taking form? I am in the process of putting one together but it is always beneficial to see how others are doing it.

I use a fairly simple one. Name, adress, number, email and a lot of blank space to draw.

You mileaage may vary, but with me, we didn't get too many custom orders a the shows. The majority came in the weeks following.

Our forms have space for
contact info
payment info
a large place to draw
a place for me to write the item they want
a place for me to fill in pricing/shipping/tax/etc
and a box which contains instructions for emailing me a photo, (which has info about what dpi as well as what kind of photo makes a good engraving, and how I might crop it etc.)
along with a disclaimer that they need to sign stating they own the photo and its not coyrighted.

it is in duplicate, and they get the original.

However, most of our orders also come a week or so after the craft shows, over the phone or email.
