I have a Dewalt router - DW618 - with a plunge base.

I have a problem with the plunge base - the top element securing the
bellows pops off when I plunge the unit - letting grease and dust in the
wrong places.

Has anyone else experienced this problem?

Do you know how to secure the bellows?

Do the bellows actually come off the end of the shaft, or is it just the cap that goes over the top end? That cap is supposed to be removable, as the dust collection connector goes there, but it shouldn't pop off on its own.

It sounds like either you still have the adjuster ring for the fixed base on or have the router rotated in the base wrong. Having the ring still on makes it really hard to get the motor into the base. The swtich should be about 180 degrees from the bellows.

The bellows come off the end of the shaft.

I can pop the cap off the end of the shaft and replace it with a vacuum adapter. That is working OK.

No - the adjuster ring has been removed, and the motor is oriented correctly in the plunge base - approx 180 degress opposite the bellows.
