Someone is asking me for a quote on part # plates for plastic molds. He wants something that will last a LONG time (I'm ruling out anodized here) and would be around 3"x5" and can be bolted to the mold. Any ideas on what material I would use for this?

stainless steel with cermark would probably outlast the mold.

I wouldn't rule out anodized. I've been using anodized for years with good success even with outdoor applications. The black is very good and is in use in many industrial applications.

I've also used Alumamark for the very application you're describing.

Tim , I do them out of laserable plastic from Rowmark and IPI. We've tested with mold cleaners and they hold up (so far). Make them with the 1/16" plastic so that they don't catch on things as easy when they are setting the mold in the press. We use a 3M double sided tape also.
Try to stay away from the Yellow with Black. It doesn't laser too good IMHO.
