One of the most practical and comfortable handles I've ever used on a router was on a $59 Ryobi 1.5hp 8.5amp from about 2001. The right handle had a great trigger switch.
They were like this:

I like the D handle because it gives me more safety and control. You have both hands on the handles when you turn the router on. Routers that have a switch on the body require that you take a hand off the handles to turn the switch on. I had a non-soft start, non-D handle router get away from me one time because of the startup torque. Ever since, I use a D handle.

Depends on the job at hand....fixed and/or D-Handle has a lower center of gravity which is good when doing dovetail or edge work. Plungers...well...are great when you need to plunge... ;)

I'll pick up a D-handle before anything else.

I have 8 with every configuration you can get but, for plunge cutting I like twin handles but, for non-plunge I like D handles.

I'm with Pat and Paul on this one but, I've never owned a D handle so I could be missing something. My Mil 5615 has the strap that allows a very controlled 'barrel' grip. that would be my preferred for DT's and edge work.
