Looking for motor wiring diagram

I am looking for a wiring diagram for this motor. Anyone have it or know where to get it. The sticker on the inside of the cover is gone!
AC motor Hawker Siddeley Electric Motors Canada Inc. 3 hp 1740 RPM F Class.

Anyone know how to hook this up. Motor must run counter clockwise when facing the outside of the pulley. The left bundle (from left to right) is T1 and T3. Right bundle (from left to right) T2, ? (light gage wire), T4, T5. T3,? and T2 are currently together. The heavy wires are T1, T2, T3, T4. T5 is med sized.55924

Just got word from EMSCO electric where I took the motor to be checked out. Motor is dead! Need new motor :( thank you for eveyone's help.

They can rewire the motor for you cheaper then you can get a new motor, and supply a wiring diagram for you at the same time.

They told me the cost of rewiring the motor would be the same as buying a new one.
