Powermatic 45 lathe question

On a Powermatic 45 lathe, is the indexing head supposed to be loose? This one rattles and clatters contuously.

The variable speed drive seems to be locked in a single speed. The belt is currently resting at the bottom of both sheaves. I know I need to replace the belt because it’s visibly worn. Is this just a belt width issue, a lubrication issue or something else? Any tips on replacing the belt?

it's been along time since I've worked on a model 45 or 90. The sheeves are adjustable as you vary the speed. If you only have one speed then the moveable half of the sheeve is frozen in place and needs to be cleaned an lubed. As I remember it's quite challenging and takes someone with some mechanical skills. I can't remember how the index is on those lathes but the index wheel should always be locked to the spindle, probably with a set screw.
