I'm not sure who manufactures these hand wheels. I'm on my third one
and this one has an enormous amount of run out as did the others. I thought
maybe the internal treads may have been threaded crooked so I chucked them
up on a metal lathe I have at work and they've all had run out.

I'm probably going to use this just as a hand wheel, but if I ever decide to
add a vacuum chuck, this isn't going to work very well.

So, should I try for number 4, or just live with it? Who knows how many i'll
go through before I get one that's true.

I have not had any issues with mine. I'm sure Nova has them made in some plant in China. Maybe they are a bad production run? I wouldn't settle. Call their customer "service" and have them select one, check it and ship it. It's the least they could do for your troubles.

Mine was good from day one (and it's one of the early China production lathes)

But Teknatool has had some uneven QA since the move to China, one batch of "X" will be fine, the next, not.
Like them having to recall the entire first run of Infinity Chucks due to improper heat treat

I've been using many Nova products for the past 9 years. I've run into occasional QA issues. Nova customer service has always come thru for me. I find their customer service to A+ and would definitely recommend you contact them. (No affiliation, just good experiences.

Thanks for the replies. Must have had one of those ( duh ) moments. It never dawned on me to contact Teknatool. Thanks again.
