Has any installed or used an electric heater that could be placed into the floor? How well did they heat? Any problems with the unit? Cost?

This sounds like you are describing what I heard called an electric floor furnace. I'm not sure that they are still sold.

Note that the output is only a little over 5000 BTU.
As for cost, at 1500 watts, take (roughly) 1.5 times your electricity cost per KWH - probably around 10¢ - for every hour of use. So, at 10¢ per KWH, it'd be 15¢ per hour to operate it.
