I used simple kitchen drawer slides for a pullout keyboard shelf I put in a computer cabinet. I added a ball detent to "lock" it in the open position. It's not adjustable, but it works very well, didn't cost much, and didn't take up much vertical space which was a big concern with the design. You can get the commercial units many places. Woodworker's Warehouse and Rockler are two sources that come to mind.

This is what I have at work, and I love it.

I don't do that much typing at my home office, so the key board sits on top of the computer desk which is a bit to high for prolonged use. The nice thing about the 3M model is that you can adjust the height. It isn't cheap, but the hardware is well made and it operates well.
