I have looked through many of my suppliers books and unable to find anyone who sells this plaque. Anyone know where or who I can purchase this from?

I would ask your local fire station captain, do you know what state this is from ?

The Department has gone through many Chief's and employees since the original purchase and no knows anything. We are located in Oregon.

Chuck, Have you tried doing an image search in Google? You may see it in one of the pictures. Then clicking on the picture should take you to the associated web site. Worth a try.

Have you thought about making it yourself? You can burn the outlines and paint the sides?
I imagine you already checked the back of the plaque to see if it had a sticker on it.
(believe it or not some don't check this first :) )

I did think about about replicating as you indicated but the FD had asked for the "exact" same plaque so I am doing due diligence and attempting to find the exact plaque.

Yes, I did look on the back and there was nothing there.

I bought a case of blank boards, so if you want to try to replicate, give me a PM and we can work out a single board.

You can buy the plate from JDS and I've bought direct from Tuff Weld in the past.

I would say that plaque was made in house.
