Looks like the majority of banding is towards the top... that screams mechanical issue to me, like belt with a flat spot, etc.

You very well may be right, it certainly could be mechanical. Guess I could pull the belts and flip them, just to see if I get a change in the pattern. Not sure much else could cause it - there is no play in the linear bearing (x axis) and the Y axis bearings (either side) move smoothly and have no play. Guess it could be the Y axis belt, just looking at the issue and pattern.

How big a piece do you need to do? If you move the image and tile to another spot, say lower right section, would the same thing happen?
