Trying to find a special chest hinge

I am trying to locate a supplier of a chest hinge like the one in this thread.

Think tackle box hinge if that helps. It looks like I could modify some designs I have seen at woodcraft and rockler, but I would love to find something already made.

I believe it's called a chest tray support. I only found it for a jewelry box but not anything bigger. It might be out there still so don't give up.
