I've noticed recently I'm not square on my cuts. I have the Incra5000 cc sled that has been dead on for some time.

So after an hour of messing w/ the miter sled I realized it had to be something different. What I found was that the blade was out of alignment with the table so as I pushed a piece through I would get a gap on the outward edge, creating a little more than 1/8" difference from one side to another on a 12" board.

I figured out how to move the entire blade setup and have gotten it much closer but still a little off but the adjustments now don't seem to be doing much for me. Is there something stupid I'm missing here?

Sound like it's time for you to do a thorough alignment on your saw. You need to make sure that your blade is running parallel to your miter track. It needs to be as close as possible. Search for the forums for table saw alignment. You should find many threads explaining how to do this.
